Circulation Proxy Borrowers Form

There are two ways to authorize a proxy borrower. You may print out this web page, fill it in, sign and submit it to the Leatherby Libraries Circulation desk.  A separate form must be submitted for each proxy user.  Or, complete the electronic submission form below.  Electronic submission forms must be submitted using your Chapman email address.  You will receive an email confirmation once the proxy borrowing privileges have been established.  For any questions regarding proxy privileges, please contact the Circulation Department at (714) 532-7723.
